Looking for PC status notifier...

Does anyone know of a server I could setup to monitor my PC and send me an email/SMS text or something if it becomes unreachable? Obviously this should be an external system running on a website or something that checks if my PC is reachable every 5 minutes or something. If it's not reachable it sends me a notification.
