TF2 Achievements fucked.

I dont know what the hell is going on with my achievements.
Two of the categories say "need title" and some of the achievements in the other categories, along with these two, just dont have names or descriptions.

How fix?

I've gotten achievements that replace the icon with the person's avatar of which I have killed to get that achievement (or maybe it was a random icon).
