Here are my brush settings.

No matter what ps brushes with 100 % brush size.

Other programs (SAI, Corel Paint tool) work entirely fine.

This occurs on both my desktop with a tablet, and my tablet pc, so my tablet is not the issue.

looks fine.

the ends are thinner, resembling less pen pressure, the centre is thicker, resembling more pressure.

you can changet eh pressure settings of your tablet in the tablets settings.

The brush looks fine, but when I actually paint it's as though it's always at 100% pressure.

google said "Hit "F1" with CS3 up and open and type in "pen pressure". You have to turn the pen pressure on and that has nothing to do with the options you list - which control different things and not the ability to utilise pen pressure. The Help Files, accessed by F1 will tell you how to turn it on."

So... try that?
