The empty slot on the toolbox is for the last tool used. Check out the shelf editor under the window/preferences/ menu. My version of Maya didn't have an add item button, but you can create your own shelves so there must be something I'm missing. You have 8.5, so it might be a bit different.

i stoped using maya at 6.5 and picked up 3dsmax 7 then 8 and i refuse to touch 9
How could you afford both 7 and 8? Unless you found the trial versions...

If you can find the tool in the menus all you have to do it ctrl-shift-click it and it will be added to the current shelf.

I found the ui vtm's at 3dbuzz really informative :) Might want to check them out. They use maya 6 in those videos but most things should be similar if not the exact same.

The First Set (http://www.3dbuzz.com/vbforum/sv_dl_list.php?c=1)

If that link doesn't work go to 3dbuzz.com and click on video training at the top. I think you have to make an account but its free. They have a set for 3dsmax too, I'm not sure how good they are though.

Easiest thing.

If you have a menu item you like -- you can press ctrl + shift then click on the menu button to assign a button to it in your custom shelf.

Even better -- it takes the last settings you have. So you can have merge set up with a threshold of 0.001, make a button, threshold of 10000000, make another button.

Click on the first button? It'll merge (weld) any vertex that's pretty close together.
Click on the second button? It'll merge (weld) all vertexes you selected to the center.

The first button is makes sure you didn't accidentally have any open edges you thought was closed. The second button is redundant if you have "collapse" which I can't seem to find in version 8.

So if you have the same tool you use with 2 settings all the time... you can make a separate button for each configuration.

Another helpful one is polySoftEdge -a 180 and polySoftEdge -a 0 for smooth or hard edge (respectively).
